Insurance Carrier Contact Information

​Policyholders may contact the insurance carriers directly to discuss filing a claim, or to update information on their policy including:
  • Change of address
  • Updating third party designee for lapse notices (your emergency contact should you miss a premium payment)
  • Request a calculation of your current benefits
Note:  Please have your Policy Number handy before calling, or be prepared to answer questions that would confirm your identity.  Spouses can usually access information without a special authorization, other family members or a non-spouse Power of Attorney (POA) may need to request a special permission form.
